New hire
Social Security number: the application asks for it (391.21(b)(1)); it is kept encrypted in the Identity card on the person’s profile, so it is not typed or printed on this form. One place to capture everything the DOT/FMCSA requires when someone is hired — the application (49 CFR 391.21), their previous employers and the safety-history inquiries (391.23), the license, medical certificate and the hire checks. It is saved to the driver’s file, so the dates show up in the DOT Compliance File and the dashboard. You can come back any time to add the answers from previous employers. Social security numbers are not entered here — keep them on the signed paper application.
This is the driver application. Office, dispatch, safety and maintenance positions: see Positions & policies (printable position sheet with the policies for each).
License
Take a clear photo of the back of the license. The barcode fills in the name, birth date, address, license number, state, class, endorsements and expiration (the same standard license-reader apps use). Nothing leaves the browser.
Accidents & traffic convictions — last 3 years
Every motor vehicle accident (391.21(b)(7)) and every conviction or forfeited bond for a traffic law violation except parking (391.21(b)(8)).
Employment history & previous-employer inquiries
List every employer for the last 3 years (391.21(b)(10)), and if the person drove a commercial vehicle, the 7 years before that too (b)(11). For every employer in the last 3 years that was subject to the FMCSRs or DOT drug and alcohol testing, you must send a safety performance history request within 30 days of hire and keep the answer — or record your good-faith effort if they do not reply (391.23). Print the request letters from the packet button below.
Medical certificate
Checks at hire
Typing the result here keeps it on the hire file only. What clears the driver for the first load is a Drug & alcohol record with the negative pre-employment result. Record it there →
